This notice explains how your health information may be used and your rights regarding that information.
Subscription Services
Your membership in the subscription program (“Subscription Services”), including your ability to access the services, begins once you complete the required intake form and your initial payment is successfully processed.
The Subscription Services may include access to licensed healthcare providers, medications where medically appropriate, online support, care coordination, and other related healthcare services.
Please note that if you are determined by a licensed medical provider to be medically ineligible for the Subscription Services prior to treatment or prescription fulfillment, you may be eligible to receive a refund in accordance with this Policy.
Participation in the Subscription Services does not guarantee any specific medical outcome, result, or effectiveness.
Cancellation Policy
You may cancel your Subscription Services at any time and for any reason.
To cancel your Subscription Services, please contact customer support at support@sabarx.com or submit a cancellation request through the chat feature available in your patient portal.
To avoid being charged for the next billing cycle, your cancellation request must be received at least seventy-two (72) hours prior to your next scheduled billing date. If your cancellation request is received less than 72 hours before your billing date, your payment method may still be charged for the upcoming billing cycle, and your cancellation will become effective at the end of that billing cycle.
Following cancellation, you will continue to have access to the Subscription Services through the remainder of your then-current billing period.
Please review the Refund Policy below to determine whether you may be eligible for any refund associated with your canceled Subscription Services.
Refund Policy
If your Subscription Services are canceled due to medical disqualification by a licensed healthcare provider prior to prescription fulfillment or treatment initiation, you may be eligible for a refund for applicable unrendered services or unprocessed charges.
Except as expressly stated in this Policy, ALL FEES AND CHARGES ARE NON-REFUNDABLE, INCLUDING FEES PAID FOR SUBSCRIPTION SERVICES, CONSULTATIONS, CARE COORDINATION, AND OTHER SERVICES PROVIDED.
Federal and state laws generally prohibit the return, refund, or reuse of prescription medications once dispensed to a patient. This policy helps protect patient safety and ensures compliance with applicable pharmacy and healthcare regulations.
If a refund is approved, refunds will be limited solely to the most recent billing cycle and will not apply retroactively to prior billing periods.
Damaged or Incorrect Items
Please inspect all medications and products immediately upon delivery. If you believe that a medication or item is damaged, defective, or incorrect, you must contact customer support promptly at support@sabarx.com.
Damaged Items
If a medication or product arrives damaged, the dispensing pharmacy may, in its sole discretion, replace the item upon verification and receipt of satisfactory evidence of the damage.
Incorrect Items
If you receive an incorrect medication or product, the dispensing pharmacy may replace the item after verification of the reported issue.
Subscription Services FAQs
What is included in my Subscription Services price?
Your Subscription Services fee represents a bundled price for the selected subscription program. Depending on the program selected, the Subscription Services may include:
- Consultations with licensed healthcare providers
- Medical evaluations conducted through live visits or electronic submissions
- Prescription fulfillment through third-party pharmacies, where medically appropriate
- Care coordination and patient support services
- Laboratory services, where applicable
- Access to support resources and patient communication tools
- Other services related to your healthcare or wellness program
The specific services, medications, laboratory testing, and support offerings included may vary based on your selected program and medical eligibility.
What happens if my provider determines I am no longer eligible for the Subscription Services?
If a licensed healthcare provider determines that you are no longer medically eligible for the Subscription Services, your subscription may be canceled. Depending on the circumstances and timing of the determination, you may be eligible for a partial refund consistent with this Policy.

Unless otherwise stated herein, the FourConcept Marketing LLC d/b/a SabaRX Terms of Service govern this Cancellation and Refund Policy.
In all cases, you remain responsible for payment for healthcare services and other services already rendered. This Cancellation and Refund Policy apply solely to Subscription Services fees and does not alter your responsibility for charges associated with healthcare services provided to you.
